Fall brings the perfect weather for a thinking walk.  This month, as the new season brings change, think about the changes you want to see in your life, beginning with a most important consideration: your health.

Where are you starting from?
Check in with your health-care team to determine your status.

Where do you want to go? Write down your health priorities and goals.

How will you get there? Jot down the actions you can take this month to meet your goals.
